Vital Records
Individuals may request certified copies of a vital record from the Clerk’s Office for a fee of:
- $15.00 for the first copy
- $6.00 for each additional copy of the same record
To obtain a certified copy of a vital record, a request form must be filled out, along with proof of identity (driver’s license, passport, or other government issued picture identification).
Proof of lineage is also required when requesting vital records for immediate family members where the applicant is not named on the record.
Identification, application and lineage requirements apply whether the records are requested in person or by mail.

Birth Certificates are available at:
- The Town in which the child was born
- The Town in which the mother was living at time of birth
- The Augusta Office of Vital Records
Marriage Certificates are available at:
- The Town in which intentions were filed and the license was issued
- The Augusta Office of Vital Records
Death Certificates are available at:
- The decedent's Town of residence at time of death
- The Town where the death occurred
- The Augusta Office of Vital Records
